If you are leaving Buffalo State, please be sure to fill out a Withdrawal From College Form available in South Wing 100. This form must be signed by your assigned advisor and the Dean of University College whose office is located in Twin Rise 100. Once the form has been completed, please submit it to the Registrar Office, located in Moot Hall. For assistance with this process please call 716-878-5223. We would greatly appreciate it if you would fill out the "Exit Interview". The Exit Interview will help us to better understand your reasons for leaving Buffalo State and assist us in better serving our students.
