Course, Program and Minor Proposal Forms and Templates
Instructive templates give all information to fill out all routing forms and the correct information needed on the form to be submitted to the CSCC for review.
Please use blue paper when printing out Course Routing forms, and use yellow for both Minor and Program Routing forms. The College Senate Office will not accept routing forms that are not on the proper paper and do not have the correct information filled in. All departments must comply with all curricular policies, both state and school. Your Associate Dean must sign and approve all submissions from their respective departments. If the Associate Dean has not signed any routing forms, they will not be accepted by the College Senate Office, and will not be forwarded to the College Senate Curriculum Committee.
