Please use this form to submit requests for User Account Setups.
NOTE: New accounts for staff and faculty must be requested by the department secretary or the user's direct supervisor.
Usernames The standard convention used to create usernames for faculty and staff is the first six letters of the user's last name, followed by their first initial and middle initial. For example, John P. Roberts would have a username of ROBERTJP. If a user does not have a middle initial then the username would be the first six letters of the last name plus first initial.
Passwords The default password for faculty and staff is set to the user's date of birth using the following format: MONDDYYYY (e.g. JAN011965 for an individual born January 1, 1965).
