IMPORTANT - Please read these instructions carefully before completing the form below.

Please use this form to submit requests for User Account Setups.

NOTE: New accounts for staff and faculty must be requested by the department secretary or the user's direct supervisor.

Usernames
The standard convention used to create usernames for faculty and staff is the first six letters of the user's last name, followed by their first initial and middle initial. For example, John P. Roberts would have a username of ROBERTJP. If a user does not have a middle initial then the username would be the first six letters of the last name plus first initial.

Passwords
The default password for faculty and staff is set to the user's date of birth using the following format:

MONDDYYYY (e.g. JAN011965 for an individual born January 1, 1965).

  *Note: This must refer to the supervising faculty or staff member or department secretary.

  **Note: NT = network logon; Exchange = E-mail (Outlook). Exchange accounts require an NT account.

  ***Note: Non-State Line (NSL) includes scholars who are visiting Buffalo State College, graduate assistants, some Research Foundation employees, and anyone else who is not being paid by New York state. Some NSL employees are not covered under BSC software licensing agreements. You will be notified if there are issues with processing the NSL request.

  ****Note: All Buffalo State College computers have a device name sticker located on top of the tower. This sticker begins with a two letter abbreviation of your building, followed by the letter "P" (for PCs) or the letter "M" (for MACs). For example, a PC located in Grover Cleveland would have a device name sticker beginning with the letters "GCP" or "GCPE." These letters will be followed by a 3 or 4 digit number. A typical device code looks like this: GCP123 or GCPE4567.
