Dear Colleague,
You may have already received a letter inviting your participation in a survey about levels of awareness of colorectal cancer (CRC) and screening methods for CRC.
If you have already responded, thank you! If not, this is a reminder to please consider participating.
The information you provided will help health educators develop materials to increase early detection of CRC among those age 40 and over. This survey will be available online until Friday, April 18.
The purpose of our survey is to gather information on your awareness of and opinions about CRC and its screening. The survey is completely anonymous and should take between 8-10 minutes to complete. Your participation will provide valuable information.
