Mid-Year Discussion

Completing the Mid-Year Discussion:  Sample Mid-Year Discussion

I.  Identifying Information 

Employee Name, Review Cycle, Budget Title, Local Title, Department, Supervisor

Name and Title, Secondary Source, Relationship, Date of review of Performance Program

II.  Progress on Goals Comments from Supervisor and 
III. Performance Feedback Comments from Supervisor 

Evaluate the employee's progress on goals and provide feedback on overall performance.  This is an opportunity to recognize and reward positive performance and if necessary, to redirect performance that is not on target.  This is not intended to take the place of day-to-day feedback but is a means of documenting feedback.  Be specific -- reference individual goals and give examples whenever possible.

IV.  The following action was taken...  

Check the appropriate box to indicate whether there have been changes in the performance program.  If changes have been made, attach a new performance program and forward it with the form.  Do not eliminate any goal that has been accomplished since it will be evaluated at the end-of-year evaluation.

V.  Signatures  

Route the mid-year discussion form to the appropriate individuals for signatures.  After the Vice President has signed it,  the form is forwarded to Human Resource Management.  The form is returned to the office of the Vice President where the original is filed and copies are made for the supervisor and employee.
