This year’s annual period in which employees may voluntarily cancel their health insurance coverage or voluntarily decline coverage for their dependents is November 1–November 30, 2008. A form is required to make this change.  Note: Unless you have a qualifying event, the 10-week waiting period still applies to enroll for health insurance for the first time, or to add previously eligible dependents to your coverage.
