Separation Checklist

Deceased Employee

  1. Notify Human Resource Management as soon as possible when notified of an employee's death.
  2. Prepare a Current Employee Change Form with a recommendation code of deceased and route for approval.
  3. Prepare a brief biography about the employee and forward to College Relations and the President's Office.
  4. Notify colleagues, staff, and if required, students.
  5. Following a reasonable time interval, notify the family or estate of personal items available at their office, desk, or work station. Contact Campus Services to arrange for assistance with packing and retrieving personal items.

Expiration of Term (Faculty and Professional)

Note: Process same as re-appointment except if recommendation is for non-renewal.

Candidates not recommended for renewal of appointment may have additional procedural rights under the collective bargaining agreement. Please consult with HRM in the case of a non-renewal of appointment.

Resignation

  1. Review the employee Separation Checklist then meet with the employee to review.
  2. Prepare a Current Employee Change Form with the recommendation code of resignation and route for approval. Attach the employee's resignation letter.
  3. Prepare a Position Release if a replacement hire is anticipated.

UUP employees shall give thirty (30) days notice of resignation (Article 32.2)

Classified service employees are expected to provide at least two (2) weeks notice of resignation.

Retirement (Faculty, Professional, and Management Confidential)

  1. Review the employee Separation Checklist then meet with the employee to review.
  2. Prepare a Current Employee Change Form with the recommendation code of retirement and route for approval. Attach the employee's letter of retirement.
  3. Prepare a Position Release if a replacement hire is anticipated.

Retirement (Classified)

Note: Retiring classified service employees must contact their retirement system (ERS) and advise them of their intention.

  1. Review the employee Separation Checklist then meet with the employee to review.
  2. Human Resources will prepare a Current Employee Change Form with the recommendation code of retirement upon receipt of the retirement notification from ERS and confirm the effective date of retirement with the Department Head/Supervisor.
  3. Prepare a Position Release if a replacement hire is anticipated.
