Notice of Vision Benefit Changes for Enrollees Represented by PEF – Effective January 1, 2009


Effective January 1, 2009, vision plan benefit changes will apply to the New York State Vision Plan for Public Employee Federation (PEF) represented New York State Employees and their enrolled dependents and to COBRA enrollees and their families with PEF represented employees’ vision care benefits.

Eyewear Purchase Period
Eligible employees will have 90 days from the date of the vision exam to purchase eyewear from a participating provider under the Occupational Program. Please note that this change does not affect the date you are next eligible to use your vision benefits.

Vision Plan Benefits
The frames and lens allowance for certain services have been increased. Refer to page 4 in the updated NYS Vision Plan Booklet (mailed to your home) for an updated schedule.

Medical Exception Program Annual Examination
The annual examination will be covered for eligible employees and dependents with a medical condition that may impact their vision refraction and who are referred by a qualified medical provider caring for that condition, regardless of whether the medical condition has caused a vision loss that requires a new prescription.

In an effort to communicate all of the plan changes, this notice along with a copy of the revised PEF Vision booklet have been mailed to you. This information is also available online at www.cs.state.ny.us. Choose Benefit Programs, then NYSHIP Online and choose your group if prompted. Select Other Benefits, then Vision to locate the link to EyeMed.

For additional information, you can also contact the EyeMed Customer Care Center at 877-226-1412 Monday through Saturday from 8:00 am to 11:00 pm ET, and Sunday from 11:00 am to 8 pm ET.
