
November 2009Health Insurance Option Transfer Period
Employees who are enrolled in the New York State Health Insurance Program
will have 30 days from the date the 2010 rates are approved and published to
change their health insurance carriers. The New York State Employee Benefits
Division will mail the new health plan rates and deadline dates to enrolled
employees’ home addresses. “Choices,” a comparison booklet of the insurance
plans, is available online. Employees may select a different health plan by
completing change forms in the Human Resource Management Office, Cleveland
Hall 410.
Important note: Univera Healthcare will not be offered under NYSHIP for the
2010 program year. Employees who are currently enrolled in Univera
Healthcare must change to the Empire Plan or a NYSHIP-approved HMO that
serves the area where they live or work during the Option Transfer Period.
Employees who are enrolled in Univera Healthcare who do not change their
health insurance carrier during the Option Transfer Period will
automatically be enrolled in the Empire Plan for the 2010 program year,
effective January 1, 2010.

is now available online. If you prefer a paper copy please contact Human Resources at 878-4821
and we will send you a copy when they become available. The Empire Plan and
HMO rates for 2010 will be mailed to your home and will be posted on our web
site as soon as they are approved. You will have 30 days from the date we
receive the rate information to make a decision.
Effective January 1, 2010, Univera Healthcare will no longer be offered under
NYSHIP. If you are enrolled in Univera Healthcare, you must enroll in The
Empire Plan or NYSHIP-approved HMO in the area where you live or work. If
you do not take action, you will automatically be enrolled in The Empire
Plan for the 2010 program year.
Please note: Blue Choice and MVP are only available to employees who reside
in the Rochester area (see specific counties in the “Choices” descriptions).
NYS Employee Health Benefits Fair
Representatives from Community Blue, Independent Health, and the Empire Plan
will be on campus from 11:00 a.m. to
2:00 p.m. Thursday, December 3, in E. H. Butler Library 210 to
answer questions about current health insurance plans and anticipated
changes in coverage for 2010. Representatives from the CSEA and UUP Benefit
Trust Funds will demonstrate how to maximize dental and vision care
benefits. Employees can have their cholesterol checked or learn about the
benefits of massage therapy. They can also obtain information about the
Employee Assistance Program and meet EAP coordinators. A representative from
Work-Life Services for New York State employees will also be on site,
offering ideas on balancing work, home, and life issues.
Changes in Health Coverage and Voluntary Cancellation of Coverage
This year’s annual period in which employees may voluntarily cancel their
health insurance coverage, voluntarily decline coverage for their
dependents, or change their pre-tax contribution is November 1–30, 2009. A
form is required to make these changes. Note: Unless employees have a
qualifying event, the 10-week waiting period still applies to enroll for
health insurance for the first time, or to add previously eligible
dependents to existing coverage. Please call Human Resource
Management at 878-4821 with questions.

NYS Flex Spending Account 2010 Open
Enrollment
Open enrollment for the 2010 NYS Flex Spending Account is Monday, September
21, through Monday, November 16, 2009. The account offers two benefits, the
Health Care Spending Account (HCSAccount) and the Dependent Care Advantage
Account (DCAAccount).
The HCSAccount allows employees to set aside from $100 to $4,000 annually in
pretax salary to pay for health-care expenses not reimbursed by insurance.
Over-the-counter (OTC) drug expenses including allergy remedies, antacids,
cold medicines, and pain relievers are also reimbursable.
The DCAAccount allows employees to set aside up to $5,000 annually for child
care, elder care, or other dependent care expenses on a pre-tax basis. An
employer contribution of up to $800 will be provided for employees in
eligible negotiating units who enroll in the DCAAccount. The employer
contribution is based on salary and is available to state employees who are
M/C or represented by CSEA, PEF, UUP, or NYSCOPBA. Pending the conclusion of
negotiations and ratified contracts, the employer contribution may also be
available to state employees in other negotiating units.
Participants currently enrolled in the HCSAccount or DCAAccount must
re-enroll to continue benefits in 2010. Re-enrollment instructions will be
sent directly from the Fringe Benefits Management Company.

Seasonal Flu Shot Clinics
10/19/09 NOTE: We have just been
informed by Independent Nursing that the Flu Shot Clinic scheduled for
October 20th has been cancelled due to the vaccine being unavailable.
Seasonal Flu Shots will be given in the Campbell Student Union Assembly
Hall on the following dates:
Thursday, October 1, 2009 from 11:00 a.m. through 3:00 p.m.
Tuesday, October 20, 2009 from 10:00 a.m. through 1:00 p.m. -
CANCELLED
No appointment is necessary. The vaccine which will be available will
protect against seasonal influenza. This is not the novel H1N1 (Swine Flu)
vaccine. The following insurances will be accepted; Community Blue,
Independent Health, Univera, Blue Choice, and Medicare Part B, provided they
show their health plan ID cards. Employees without proper insurance coverage
will be charged $30 for the flu shot. If you have any questions regarding
your health insurance coverage, please contact Employee Benefits at ext.
4821, or call member services of your insurance company. The phone number
should be on your insurance card.
